Apple Ridge Family Medicine is not accepting new patients at this time. Please check back with us in January 2024.
For your first visit with us, please bring along the following:

The medicine you are currently taking

Any other pertinent information needed
We ask that you arrive 15 minutes prior to your appointment and that you provide 24 hours' notice if you need to cancel or reschedule.

New Patient Online Form
Please use our secure online form to complete the new patient paperwork, prior to your appointment. Failure to complete the form will result in the rescheduling of your appointment. If you have any questions about completing the forms, please give us a call at (717) 334-8165.
Please be sure to complete the record release section of the online form. We will need to obtain a copy of your medical records in order to provide you with the highest level of health care.
Medical Marijuana
We are currently accepting Medical Marijuana patients. Please use our secure online form to complete the paperwork for the program. On this site, you can book an appointment at a time that suits your schedule, pay for your visit, as well as get reminders via email and text. Please note, you must complete your state registration with the PA department of health prior to scheduling our appointment.
